Book a Health History


The Health History is an invitation to the health coaching relationship. Let’s get to know each other and find out if health coaching with me is right for you.

All prospective clients must complete the Health History Questionnaire and book a session before working with me in my health coaching practice. During our time together, you'll have an experience of what it's like to work with me while we discuss your health goals and decide whether health coaching with me is the best option for you.

The Health History Questionnaire will take about 20 minutes to complete, and our session will run for 60-75 minutes.

If we decide we're a good fit, the cost of your session will be applied to the coaching package offered to you. You'll have three months to apply your session to the offer. After that, you'll have to pay at the full investment level to join.

The purpose of the Health History is not to judge, diagnose, treat, heal, prescribe, or cure, but for me to have a complete understanding of your current situation so that I can offer you the best and most comprehensive support for your unique concerns and lifestyle goals.  Your information is confidential and will not be shared with anyone without your informed and written consent. 

Your Health History must be submitted at least two days prior to your scheduled session so that I have adequate time to prepare, and you must schedule your session within three weeks of submitting your payment.

Refund and Cancellation Policy

Cancellations and rescheduled appointments are honored for changes made within 48 hours or longer of booking.

I do not offer refunds for

  • late or missed sessions
  • last minute cancellations (24 hours or less)
  • you changing your mind
  • sessions not scheduled within three weeks of submitting payment

Please make your decision wisely, factoring in your ability to invest at this level without fear or worry about your finances or well-being.
